A big part of ensuring you have a great experience at Restorative is to know what to expect before you come in for an evaluation and begin treatment. Whether it’s the initial evaluation stage—when your therapist conducts an assessment of your injury, condition or pain—or the treatment stage, when you will actually be diving into symptom relief, we want you to be informed every step of the way.

Our team looks forward to welcoming you to our clinic and taking care of you from the moment we connect. We will assist you with all aspects of your care experience from insurance verification to appointment scheduling and billing.

Please print and complete the new patient forms prior to your first visit. This will allow you to quickly get started with your program and be on the road to recovery. If you cannot print and complete the forms before your first visit, please arrive 15 minutes before your scheduled appointment to complete the necessary information. You can find the forms here.

Arrival

We encourage you to print out the new patient forms prior to coming to your appointment. If you are unable to do that, we encourage you to arrive 15 minutes prior to your scheduled appointment time to submit your paperwork and provide your insurance information.

What should I bring?

You will need to bring the following items for your first appointment:

  • Photo identification - accepted forms include:

    • Valid driver’s license

    • Valid state ID

    • School ID card

    • Valid passport

  • Prescription/doctor referral

  • Insurance card

What should I wear?

We recommend that you wear comfortable clothing that allows you to move around easily for your appointments. Athletic wear, workout attire and gym shoes will work well.

What can I expect at my first appointment?

You can expect your first visit to last for about 60 minutes. Upon arrival, you will be greeted by our Patient Care Coordinator who will ask to see your photo ID, insurance card and any other paperwork that you brought with you. If you were unable to complete the new patient forms, your PCC will provide those to you at this time to complete prior to meeting with your therapist.

Once you are checked in for your visit, your therapist will bring you to a private exam room where they will complete a comprehensive evaluation as well as review your medical history, perform diagnostic testing and speak with you to learn more about your condition.
